This survey will help us target our new products and promotions to what the salon professional needs and is looking for  ... YOU must be a salon professional to participate in this survey -  if you are a winner in the drawing --We will require a faxed copy of your license or business card with current address before shipping your prize.

Rules: Please read carefully...
Remember - none of the "contact info" is given to the manufacturer unless you win the prize.

  • We MUST have your correct email address - because this form is handled by a form handler we can not see your email in the "from" line.  It is very important you enter your email correctly.  If we can not contact you via email we will award the prize to another survey respondent.
  • You must answer every question in the questionnaire - in detail - if it does not apply to you -- enter-- "does not apply" or something similar.
