Your name, mailing and email addresses will not be given to the survey sponsor.  Just the info you fill out in regards to the questions they ask & type of beauty professional your are will be sent to the manufacturer.   Beauty Web does (however) need you to fill out your name and email address for our records so we can select a winner and notify you.

Obviously the winners name and address will be given to the manufacturer so they can ship you the prize.

Help improve the products and services you use by sharing your likes and dislikes with the people responsible for the products and services you use.

We will post the winners as the drawings are held - only one entry per member.  If you enter twice you will be disqualified.  We will inform you by e-mail if you are a winner, so please enter you FULL and Correct email address.
